What is treatment-resistant depression?
Treatment-resistant depression usually means that a person has not improved enough after trying more than one standard treatment. This can include antidepressants, psychotherapy, or both. TRD is not a sign that someone has failed. It means the condition has been difficult to treat with the options tried so far.

What did the new 2026 NHS study find?
A 2026 randomized, double-blind study in an English NHS setting included 60 people with major depression that had not responded well to previous treatment. Participants received either a single 25 mg dose of psilocybin or placebo. Importantly, the psilocybin was not given on its own. The program included preparation before the session, support during the dosing day, and integration afterwards. The psilocybin group showed a larger fall in depression scores than the placebo group. At three weeks, 43% met criteria for response and 40% for remission. At six weeks, response was 50%. These results are promising, although the study was still relatively small.

What did the 2026 meta-analysis show?
A living systematic review published in Nature Mental Health included 15 randomized trials with 801 participants. Twelve trials were included in the main meta-analysis. Overall, psilocybin was linked with a large reduction in depressive symptoms compared with control conditions. The authors also warned that many studies are small and some have risk of bias. That balance is important: the evidence is promising, but the field is still developing.

Why can one supported psychedelic session be different from daily medication?
Most antidepressants are taken every day. Psilocybin-assisted therapy is studied in a different way. The psychedelic experience may create a short period in which emotions, memories and fixed thought patterns can be experienced differently. A person may see old problems from a new angle. That does not mean one dose is a cure. It means one carefully supported experience may create an opportunity for therapeutic work.
